What is Philhealth ID
Anyone who registers for a PhilHealth membership receives a special, permanent number called a PhilHealth Identification Number (sometimes known as a PIN or PhilHealth ID number). This is used for member identification, determining whether they are eligible for PhilHealth benefits, and keeping track of when they use those benefits.

Upon enrolling in PhilHealth, you receive an identification card in addition to your PhilHealth ID number. This identification card attests to the cardholder’s and their qualified dependents’ eligibility for the National Health Insurance Program’s benefits and privileges.
Philhealth requirements: Step-by-step guide on how to apply
1. Fill-out two (2) copies of the PMRF
First and foremost, you would need to fill out two copies of the Philhealth Membership Registration Form. Thanks to the internet, you can download it here.
2. Prepare your Philhealth requirements
The following documents are part of the Philhealth requirements for your registration:
- Two (2) 1×1 latest ID picture
- Two valid IDs
- Any of the following documents:
- SSS pensioners
- Death, Disability and Retirement (DDR) photocopy indicating the date of retirement and effective date of pension
- Retiree/Pensioner Certification photocopy indicating the effective date of retirement
- For GSIS pensioners
- Certification/Letter of Approval of Retirement photocopy indicating the effective date of retirement
- Service Record photocopy issued by the employer showing rendered services of not less than 120 months (10 years)
- Certification/Retirement Gratuity photocopy from the employer indicating services of not less than 120 months
- Photocopy of retirement voucher issued by GSIS
- For Uniformed personnel (AFP, PNP, BJMP and BFP)
- General/Special or Bureau Order photocopy indicating effectivity date of retirement
- Certification/Letter of Approval of Retirement photocopy from the GSIS indicating services of not less than 120 months
- Statement of Services photocopy issued by previous employer showing service of not less than 120 months
- GSIS Disability Pensioner / SSS Permanent Total Disability Pensioner before March 4, 1995
- Death, Disability and Retirement (DDR) photocopy indicating the date of retirement and effective date of pension
- Disability Pensioner Certification photocopy issued by SSS/GSIS indicating effective date of pension or the period of coverage for disabled pensioner.
- SSS Survivorship Pensioner before March 4, 1995
- Death, Disability and Retirement photocopy indicating the type of survivorship in nature and the effective date of pension
- Survivorship Pensioner Certification photocopy indicating the effective date of pension
- Others
- Photocopy of official receipts of premium payments to Philhealth
- Any other documents indicating the months of premium payments to Philhealth
3. Submission
Once completed, you may proceed to the nearest Local Health Insurance Office (LHIO) to submit the duly accomplished PMRF and necessary documents. To know the nearest LHIO in your area, you may check Philhealth’s Directory of Offices here.
Once submitted, you just have to wait for the Member Data Record (MDR) and Philhealth ID card.

How to get Philhealth ID
For employed
Expect your HR officer to process your PhilHealth membership and give you an ID if this is your first job in a private company and you don’t currently have a PhilHealth number. Here is the standard procedure:
- Fill out two copies of the PMRF.
- Submit the PMRF to your HR Department.
- Wait for your employer to give you your Member Data Record (MDR) and PhilHealth ID card.
For self-employed and unemployed
If you’re self-employed or unemployed, you must first sign up as a voluntary member before requesting a PhilHealth ID card. Here’s how to apply for Philhealth ID:
- Fill out two copies of the PMRF.
- Go to the nearest Local Health Insurance Offices or PhilHealth Express outlet.
- Submit the PMRF.
